Dental referral process.
                                                              
Referring a dental case is as easy as 1, 2, 3. The transition between the patient and client from your office to mine and back to yours will be seamless. 
 
The easy 3-step process:
 
            1.      Give the owner a copy of “A referral to Dr. Scott -What to expect”.
 
2.      Give me a call to discuss the case. Office or cell phone is fine.
 
3.      Fax a completed “Dental Referral Request and History Form” and any lab work you have done to 888-387-2173 OR go to my website and fill out my online referral form at: http://artoothvet.com/site/view/153345_OnlineReferralForm.pml.  That’s all you do. I will take it from there.
 
Following the appointment the owner will get discharge instructions, photos, radiographs, etc. and a client education sheet that’s case specific.
 
I will mail or e-mail you a letter that states the findings, diagnosis, treatment and follow up, a copy of the dental chart, copy of the dental radiographs, photos, and any other information related to the case. For cases where biopsies were taken, I will contact the owner with the results and mail you a copy of the histopathology report. I will also handle any follow up with the client and keep you informed via mail and/or phone.
 
Recheck appointments are typically scheduled as follows for the indicated procedures:
 
            7-14 days------ Oral surgery, orthodontics
            1-2 months---- Periodontal treatments
            6 months------ Root canal treatments
            12 months----- 2nd recheck of root canal treatment
 
I do all follow-up treatments and recheck appointments. If the owners live a considerable distance from the office, I will recommend they take their pet back to you for a recheck of the surgery site. If there are problems, you can let me know.
